六旬病患进食噎着辞世 院方未及时换软食引验尸官关注

一名病患在黄廷方医院拔牙后,牙科医生建议医疗团队为病患准备软食物方便进食。不料病患仍食入普通餐,结果噎着而昏迷,不久即辞世。

上述个案发生于去年一月底。在昨日就辞世病患死因展开的研讯,验尸官马文德对于医疗团队并没有立即执行牙医建议,为病患更换软食表示关注。

据《海峡时报》报导,去年1月11日,因为腰部及膝盖剧烈疼痛致不能行走,67岁的病患Simon Lee被救护车送往黄廷方医院接受治疗。

医生诊断Simon Lee患有链球菌菌血症(streptococcus bacteremia),怀疑病情是因为口腔问题造成,要他住院治疗。

医院的牙科顾问医生在1月16日检查病患口腔,决定为他拔出三颗严重蛀牙的牙齿。病患在拔牙后只剩下九颗牙齿,牙科医生建议医疗团队将他的饮食换为软食物,让他较易进食。

牙医的建议也记录在电子医疗报告中。

不过,手术后Simon Lee仍继续食用普通餐,直到手术隔日,护士听到病患剧烈咳嗽,上前查看询问但他并没有回应,但她事后声称并没有发现他噎着。

Simon Lee 对面病床的病人见状,立即走到病房外找医生求助。医生到了病房时,发现Simon Lee已经倒下,尝试清理他的食道,过程中见到食物残留物。

医生见病患失去了脉搏,一边做心肺复苏一边将他送进加护病房,虽然他在17分钟后恢复了心跳,但是却昏迷不醒。

医生进行了支气管镜检(bronchoscopy),发现他肺部有大量的食物残留物。病患的家人在去年1月28日决定拔管,他最终在2月18日去世。

病患去世后,法医确认他的死因是脑缺氧,以及气管被异物阻塞所导致的肺炎。

在研讯上,验尸官马文德认为这是一起不幸意外,“尽管如此,牙医Bertrand Chew博士提出的饮食建议还没有立即实施。”

他提到,病患在经过牙科手术后,显然处在较脆弱的状态,更有必要适当地预防窒息的潜在风险。

不过,验尸官表示就目前呈现的证据来看,仍未能断定没有更换软食,就是导致病患窒息的主因。这是因为病患还患有其他严重慢性疾病,如中风、心脏病和时常剧烈咳嗽等。

事实上,在昏迷前,病患在手术后的16日和17日仍能正常进食普通餐。

验尸官也表示,事故后院方采取必要措施,提高医生对病患的比例,也加速处理医生的指示。院方则对病患家人致哀。

至于出席昨日研讯的病患儿子,受访时不愿置评。
