医疗费用大增,为何保健基金的支出却下降?

我针对报章的《11月通膨率达4.2%,创25年新高》(海峡时报,12月25日)和新闻报道“新加坡11月份消费指数年比提高到4.2%”(亚洲新闻台,12月24日)。

比起其他的费用,医疗费用的通膨居首是6.2%,其次是交通和通讯5.6%,其余才是4.2%。

消费者物价指数——一个非核心物品和服务收费的测量——继10月份的3.6%后,又攀到4.2%的水平。

既然医疗费用的通膨是最高的,那么像保健基金这类对穷人的援助是否也水涨船高呢?

我发现薛昌宁(音译)的这篇文章非常有趣《检视南方:公众对近日政策不满》(星报,马来西亚报章,12月22日)

他的文章说:

“尽管李光耀资政信誓旦旦预测接下来将是黄金盛世,新加坡的中产阶级国人依然对政府持否定态度。

“最近这个通膨是12年来最坏的一个,人民多认为一部分是因为政府调高公共服务费用,从保健到印花税无一幸免所引起的。”


我也注意到另一篇报道《30万1000名急需病患获得保健基金援助,创历史新高》(海峡时报,12月4日)

保健基金的人均支出在2005年和2006年分别是136新元和132新元。

2001年的人均支出则是174新元。

既然医疗费用的通膨是最高的,为什么保健基金的人均支出却出现下降的趋势?2006年和2005年比是下降了3%。2006年和2001年比是下降了24%。

为了更好地说明通膨压力,我举个例子:平均住院账单以2005年跟去年(2006年)同比是涨了10到30%,特别是接受大量补贴的C级病房,涨幅最大。

经济上,政府说去年是前所未见的好年头,并且失业率也创新低,为什么需要保健基金援助的病患却激增呢?

为什么被保健基金拒绝的申请者也从2001年210位,翻了30倍至6456位呢?

保健基金的申请准则是否跟着上升的医药费和通膨率而调整呢?

在我为贫困新加坡人提供的财务指导的义务工作里,每当我问及医药社工有关保健基金的申请准则时,他们都会告诉我:“这是机密。”

我也不能在任何政府网站,或者福利组织的网站获得任何的资讯。

为什么保健基金的准则会是个机密呢?

院方是采取哪些“收入调查”的构成来评审一项保健基金申请?

最近我在电视上看了一个慈善筹款的演出,晚会利用新加坡老年人在镜头前呼吁捐款,因为这些人没有钱支付医药费。我的一个朋友的姐姐,五十多岁的家庭主妇,在隔天,从自己10楼的组屋住家,一跃而下。

在一项新加坡老化问题的研究里,新加坡人最担忧的就是医药费。

海峡时报的另一篇文章《安老院退出保健基金:院方认为和照顾病患的原则相违背》(海峡时报,12月27日),这篇报道说:“安老院不希望垂死病患和他们的家属耗尽最后的储蓄,仅仅是为了证明他们符合资格。”“我们的一些病患病得很严重,来到我们这里,离他们死去只不过是几天的时间,看到家属这样已经够难过了,我们不希望他们再经历一次无谓的调查过程。”

同篇文章也说:“去年,保健基金发出巨额3千960万新元给大约2万至3万名病患,总数为30万1126项合格申请。”我相信这是首次在媒体上看到保健基金受惠者的数目。

我也想知道2001、2005和2006年合格申请和拒绝的人数。

2006年保健基金拒绝的6456这个数目,是病患人数还是申请项目?如果是病患人数的话,是不是意味着有26%的人被拒绝了(6456除以2万5000人)?

综观上述所议,我建议保健基金的申请准则必须公开,那么那些需要援助的新加坡人就不必为医药费过分担忧,甚至焦虑了。
