国人最担忧罹癌 惟少于半数进行癌诊测

跨国医疗管理公司Medix公布一项问卷调查,探讨新加坡人民对于求诊和接受治疗的态度。几乎大部分受访群体对于个人健康持感到悲观,也惧怕被诊断出患有癌症。

许多新加坡人对征询第二意见和进行体检十分积极,也更渴望获得较个人化的治疗建议和选择。

研究显示,相比32巴仙的女性,有53巴仙男性更担心中风来袭;癌症则是两性最大的担忧,其次为心脏病(51巴仙)和糖尿病(44巴仙)。

少过半数人进行癌症诊测

这份问卷调查,乃是委托市场研究公司明略行(Millward Brown)。于2018年8月进行,透过网路采访600名年龄介于26至59岁、收入中等或中上的男性与女性。

虽然惧怕罹患癌症,但只有不到半数人会进行检测癌症的早期诊断。仅37巴仙女性受访者进行乳房和子宫抹片检查;此外,仅29巴仙受访者进行肝功能测试;21巴仙进行甲状腺功能检查。这种情况令人担忧。

Medix总裁Sigal Atzmon 认为,调查的重要发现,乃是民众恐惧罹患癌症,却对参与诊测欠积极。只有超过半数受访者找肿瘤专家咨询抗癌;此外,民众对于该采取哪些重要的步骤和如何选择合适的疗法,缺乏认知。

只有20巴仙人士知道,在进行治疗前需进行组织检验;仅12巴仙知道癌症分期(staging)用以判断癌症发展与扩散程度,两者都与抗癌成果和存活率息息相关。

不过,国人普遍愿咨询第二意见,也乐意善用新科技抗癌,惟Atzmon 预见有必要推行更积极的还教育群众工作。

Atzmon认为,对于国内的保健选项国人想了解得更多。应付健康问题最好的方案,就是做好预防措施、定期体检、预备额外的医疗选择,以及去咨询合适的专科。

“知识就是力量”,他补充,保险业者、医生和政府,都应该提供民众触手可及的必要资源,让国人有知识、客观地作出选择,乃首要任务。

调查显示,只有58巴仙受访者认为,他们的医生传授足够的预防疾病资讯。有87巴仙在接受严重疾病诊断时,都会再征询第二意见。

78巴仙受访者同意进行基因测试的重要,60巴仙也认同需定期体检。受访者们也希望获得更多有关治疗和预防的资讯。

有78巴仙对于针对严重疾病的个人化医疗管理感兴趣,86巴仙则偏好跨专科的治疗法。高达91巴仙受访者会搜寻有关治疗癌症的新技术资讯。

有41巴仙受访者在寻找专科诊疗,都会请教医生介绍,而14巴仙则上网搜寻。四分之三人士对专科医生提供的意见感到满意。46巴仙则认为,选择专科和医院对于医疗结果来说同等重要。

至于对公共和私立医院的满意度则旗鼓相当,分别达到67与68巴仙。

不鼓励中西合璧抗癌

在实际治疗癌症方面,有42巴仙人士混合中医和西医的疗法。Medix全球医疗总监David Zelster教授感到担忧,这是因为医学文献和研究表明,若没有适当建议下,结合传统中医药和西医治疗恐会为癌患带来副作用。

例如,中医常用的人参有助预防癌症和恢复体力,但同时诱导细胞色素P450 3A(简称CYP3A)的活跃,致使使用化疗药物的成效降低。

对此,医生应更主动咨询病患病史,包括了解其食用的草药并对于合适的用量提供建议。

Zelster教授认为,从此次问卷调查结果,加上Medix公司对个人医疗案例管理服务的经验,给国人提出应对癌症的指南:

  • 依据个人疾病风险和家庭病史进行体检
  • 开始治疗前进行活体检验和癌症分期诊断
  • 咨询肿瘤专家,也咨询其他的医学意见或跨专科的治疗法。进行额外医疗征询或医疗管理,都要索讨自身的医疗信息,这对于保障有素质的治疗过程很重要。
  • 查询是否有新的治疗方法、对肿瘤进行基因测试,藉此个性化您的治疗方案
  • 要使用传统中医疗法应先咨询肿瘤专家的意见

吁政府确保癌诊测普及化

最后,Atzmon呼吁新加坡政府,应更积极教育民众,有关预防和治疗癌症的方法,并改善获津贴的癌诊测服务,使之更为大众化。

在私立医院的治疗费用也应设法调低,例如透过配套式定价等等。同时,私人界的医疗透明化和治疗报告的反馈,也亟需加强。
